What you will be doing

Location: Reading, PA but open to Relocation (will provide relocation compensation)

Sign-On Bonus: $10,000

This position is 100% on-site at Reading Hospital, located in West Reading, Pennsylvania. Typical work hours are M-F between 8 AM and 5 PM.


Cencora has managed and staffed the Reading Hospital Ambulatory Pharmacy inside of the 700 bed Reading Hospital since 2017. In 2023 a second community pharmacy will be opened to serve the Doctors Office Building located on campus.

We foster a culture where career development and training play a key role as our staff gain experience in many aspects of ambulatory and specialty pharmacy. We currently employ pharmacists in both clinical and ambulatory settings with a support staff of technicians in various roles. We are excited to welcome new team members to continue to grow our team and kickstart new careers.

We started with 2 pharmacists and 4 technicians and have since expanded into a much larger operation with services such as:

  • meds-to-beds
  • employee prescriptions
  • pharmacist navigators embedded in clinics
  • Specialty Pharmacy

This position is in an ambulatory clinic and involves direct patient care and collaboration with other healthcare providers. A strong clinical pharmacy background with an understanding of retail pharmacy operations and prescription processing is optimal to succeed in this role.

  • Demonstrate value of ambulatory pharmacy partnership to physician practices.
  • Maintain presence in clinic, field medication questions from providers, such as formulary product selection, product dosing, ADR's, and other considerations.
  • Ensure prescriptions flow through ambulatory pharmacy and are identified by pharmacy staff.
  • Maintain patient list, schedule appropriate follow-ups to assess medication safety and efficacy, communicate treatment plan recommendations with provider.
  • Coordinate patient refills. Ensure auditable records showing proof-of-delivery are maintained.
  • Prepare and submit prior authorizations, manage denials/appeals.
  • Triage prescriptions which are limited distribution drugs or out-of-network.
  • Maintain patient schedule for clinical pharmacist services including, but not limited to, medication counseling, pillbox refills, immunization, medication administration.
  • Act as financial advocate for patient when needed, leverage charitable foundations, state programs, and/or manufacturer programs to ensure medication affordability.
  • Demonstrate ability to navigate various platforms and document appropriately in EMR and specialty therapy management platform.
  • Contribute to expansion of ambulatory pharmacy services through active participation in navigator committee and projects.
  • Cross-train to ambulatory pharmacy, assist with staffing, as needed.

What your background should look like

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • PharmD degree required.

  • PGY1 residency or previous experience in ambulatory care is highly desirable.

  • Unrestricted pharmacist license (incl. immunization) in the state of PA is required. If out of state, must obtain licensure within 90 days of receiving offer letter.

  • Experience with 340B purchasing program is beneficial.

MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:

  • Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint.

  • Experience in Epic or similar EMR platforms.

  • Experience with retail pharmacy prescription processing and claims adjudication.

  • This role involves working with broad array of complex disease states and medical conditions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records.
