DESCRIPTION SOMMAIRE DU POSTE :

L’Infirmière/infirmier d’injection et de perfusion sur le terrain (infirmière/infirmier sur le terrain) fournit des services spécialisés de traitement par infusion et injection et des services infirmiers aux patients non hospitalisés atteints de cancer, de maladies auto-immunes ou rares, en clinique ou à domicile. Une coordination et des communications efficaces jouent un rôle essentiel dans les services aux patients, aux aidants et à nos clients. Le titulaire de ce poste offre aux patients et aux fournisseurs de soins de santé des formations et un soutien continu relativement aux maladies et aux traitements qu’ils gèrent.

PRINCIPALES FONCTIONS ET RESPONSABILITÉS :

– Administrer les médicaments (par voie IV, IM et SC) au domicile du patient ou dans une clinique spécialisée

– Surveiller les réactions post-injection/perfusion, noter et déclarer toutes les activités conformément aux protocoles des programmes

– Fournir des formations aux patients sur les protocoles des programmes, individuellement ou en groupe.

– Signalera les évènements indésirables, les indésirables graves, les plaintes relatives aux produits conformément a la formation et aux politiques et procédures d'Innomar.

– Offrir des disponibilités régulières et prévisibles pour répondre aux besoins de planification des rendez-vous de façon efficace et rentable et fournir aux patients un accès rapide aux traitements nécessaires.

– Coordonner quotidiennement avec les patients, les gestionnaires de cas, les médecins, la Planification des rendez-vous et les autres départements pour définir le calendrier de planification des rendez-vous, les traitements et la communication de renseignements.

– Participer régulièrement aux consultations téléphoniques, aux séances de formation et aux réunions sur le Web avec le gestionnaire de district pour partager des renseignements et respecter les normes de pratique professionnelle.

– Consulter avec les médecins, les clients des domaines pharmaceutiques et biotechnologiques au sujet de l’éducation et de la mise en œuvre de programmes propres à certains produits.

– Respecter les normes de pratique infirmière professionnelles établies par les associations professionnelles et la compagnie.

– Travailler en collaboration avec les équipes de santé multidisciplinaires en assurant la surveillance et la délégation des tâches applicables aux IAP/IAA selon les autorisations des ordres professionnels provinciaux.

– Effectuer les tâches et les fonctions connexes au besoin.

EXPÉRIENCE ET NIVEAU D’ÉDUCATION REQUIS :

– Infirmière autorisée qualifiée (avec un permis valide de l’Ordre provincial des infirmiers et infirmières)

– Certificat de RCR de prestataire de soins de santé requis.

– Au moins trois années d’expérience comme infirmière autorisée

– Un an d’expérience de l’administration d’intraveineuses et de perfusion.

– Expérience des soins critiques, d’urgence, de soins de courte durée aux patients hospitalisés ou non ou des soins à domicile est considérée comme un atout.

– Une certification ACLS ou ACAV et de l’expérience des voies veineuses centrales sont des atouts

– Maîtrise de MS Excel, Outlook (courriel) et MS Word.

– Accès à un ordinateur à domicile avec une connexion internet, une imprimante, un télécopieur et un
Téléphone cellulaire pour prendre et confirmer les rendez-vous et les affectations.

– Souplesse afin de fournir les services pour les rendez-vous de patients planifiés et les rendez-vous occasionnels de dernière minute.

– Excellentes aptitudes du service à la clientèle pour communiquer avec les patients, les médecins et nos clients.

– Excellentes aptitudes de communication écrite et verbale

– Capacité à travailler de façon autonome avec peu de supervision dans le cadre d’une grande équipe à distance.

– Capacité à se déplacer vers les cliniques et pour les rendez-vous à domicile dans un secteur géographique défini.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Administer medications in the patient's home, specialty clinics and Physician Partnership clinic settings.
  • Assess monitor pre and post-injection/infusion treatments as directed by program protocols.
  • Completes necessary documentation based on policies and procedures, and as directed by the College of Nurses, and Innomar ISO accreditation standards of practice.
  • Provide patient / Caregivers education based on product monographs and program protocols, individually or in groups
  • Report adverse events, serious adverse events, product complaints in accordance with Innomar training and policies & procedures
  • Provides reliable availability based on the agreed upon employment agreement to meet the needs of the IBU and the Nursing and clinics Key Performance Indicators.
  • Communicates and works effectively amongst multi-disciplinary teams i.e. case managers, physicians, pharmacy, scheduling and other departments to meet established timelines for scheduling appointments and treatments.
  • Communicates effectively with Patients/Caregivers to set up appointments and drug delivery to home and clinic.
  • Participate regularly in team web-based meetings/training sessions with the Business Area Manager, RNE and Nurse Champion to maintain and enhance professional practice standards.
  • Participates in specialized client related trainings as required.
  • Adhere to professional nursing standards as prescribed by the professional associations and Innomar.
  • Work collaboratively within multidisciplinary health care team providing oversight and delegation of applicable tasks to RPN/LPN as authorized by provincial college of nurses
  • Required to travel within your designated IBU and surrounding IBUs to meet the operational needs.
  • Required to follow all nursing and clinic standard operating procedures and Work Instructions.
  • Required to follow all Health and Safety standards as per the provincial Health and safety Act.
  • Required to complete ongoing and annual education training as per the training matrix within the specified timeframes.
  • Perform other related duties and tasks as required by Innomar.
  • May be required to be on standby/on call to meet the service needs.

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • Relevant experience as a Registered Nurse required
  • 1 year experience IV initiation experience
  • Current registration with licensing body; CPR certificate
  • Experience in a critical care, emergency, acute care inpatient/outpatient or homecare setting
  • ACLS, PALs, MS and CVAA Certification, and central venous lines/port experience are considered assets
  • Proficient in MS Excel, Outlook (e-mail), and MS Word and other applicable computer programs.
  • Access to internet connection,
  • Knowledge of Cell phone for making and confirming appointments/assignments and fax machines.
  • Excellent customer service skills for managing daily interactions with: patients, family members, physicians and our clients. Strong written and verbal communication skills
  • Able to work autonomously with little supervision as part of a larger team
  • Able to travel to clinics and homecare appointments within a set geographical boundary

MINIMUM SKILLS, KNOWLEDGE AND ABILITY REQUIREMENTS:

  • Advanced knowledge of pharmaceutical distribution industry is an asset.
